to the paper forms provided by the school. Please
contact the Financial Aid office, if you have

1: Itemize Your Monthly Expenses
| This is the first (1st) of three (3) forms to
be completed in the first year. There are a lot of expenses not
directly related to educational costs that you
still have to take into consideration. Some of
these expenses are unavoidable, others reflect
personal spending habits. Please take your time
when filling out this form - plan ahead ! You
may budget monies to reimburse yourself for books,
health insurance and the $750.00 building fund
fee. However, you are responsible for making these
payments at registration. |

| This is the second (2nd) of three (3) forms to
be completed in the first year. |

| This is the third (3rd) of three (3) forms to
be completed in the first year. |
